Cleveland Institute of Art ID cards are issued through the Case Western Reserve University Access Services Department located in Crawford Hall on Euclid Avenue. The first ID card is issued free-of-charge. Replacement IDs will cost $25 each time the card must be replaced. For a replacement ID, students are required to obtain a yellow ID permit card from the Institute’s Registrar’s Office, which must be presented to CWRU Access Services along with the $25 fee.
Students withdrawing from the Institute are required to turn in their ID card to the Registrar's Office at the time of withdrawal.
Current students should complete the change of address and name form and return it to the Registrar’s Office. If changing your name, you also must bring a copy of your social security card or the court order with your legal name change.
